Important Documents

When preparing for admission day, having all necessary documents organized and ready is crucial. Important documents ensure that the admission process goes smoothly and that the staff at Alpine Fireside Health Center can provide the best possible care from the very beginning. This list will outline the key documents you need to bring, helping to streamline your admission and avoid any unnecessary delays.

  1. Copies of Power of Attorney for Health Care, Power of Attorney for Property, Living Will, Surrogacy and Guardianship papers.  
    Please note! If a resident has advanced directives inplace, the facility must have a copy of the document for the medical record at the time of admission. The policy of this facility is: a resident will be treated as full code: life sustaining measures will be used, if advance directives are not in place.
  2. Medicare card, social security card, health insurance card, Medicare supplemental insurance card.
  3. Copy of long term care insurance policy (if applicable)

What to Bring for Your Residents needs

All clothing must be marked on an inside seam or tag prior to resident’s admission. Bring a laudry basket or bag for soiled laundry. 

Please note that the quantities listed are suggested amounts only:

  • 14 Pairs of underwear
  • 14 Bras/undershirts
  • 14 Pairs of Pajamas or night gowns and Bathrobe
  • 1-2 Pairs of slippers (nonslip bottoms please)
  • 14 Pairs of socks/panty hose
  • 14 Pairs of pants
  • 14 Shirts/Blouses
  • 14 Dresses (if preferred to  pants outfits)
  • 14 Sweaters
  • 14 Sweat shirts or sweat suits
  • 2-3 Belts, pairs of suspenders
  • 2-3 Pairs of shoes (washable, non slip bottoms please)
  • 1 Winter coat (hat, scarf, gloves) & 1 light weight jacket

***Label all clothing prior to admission.
***Towels and Linens are provided and laundered by the facility.
***Incontinence pads are provided by responsible party or family.
***Pillows provided by the facility or if you prefer, you may bring your own.
***Rooms are furnished with a regular single bed, a 3 drawer nightstand, a 3 drawer dresser, and a high back chair. You may bring your own favorite pieces, if preferred.
